By accessing or using the website located at www.cascadepharmacy.com ("Site"), you agree to be bound by these Terms of Service ("Terms"). If you do not agree to these Terms, please do not use this Site.

These Terms apply to all visitors, patients, and users of the Site. They do not govern the pharmacy services we provide in person, which are subject to applicable federal and Oregon state pharmacy law and our patient agreements.

This Site is operated by Cascade Pharmacy, a licensed retail pharmacy located at 7591 Crater Lake Hwy, Suite A, White City, OR 97503. The Site is provided for informational purposes and as a convenience for our existing and prospective patients in the Rogue Valley area.

This website does not constitute a pharmacy, a medical practice, or a healthcare provider. All pharmacy services are provided in person at our licensed physical location.

This Site does not facilitate, enable, or provide:

• The online purchase of prescription medications
• The online ordering or dispensing of controlled substances or prescription drugs
• Internet-based prescribing or telemedicine services
• Mail-order pharmacy services
• The shipment of prescription medications to addresses outside our Rogue Valley service area

All prescriptions are filled by a licensed pharmacist at our physical location and are available for in-store pickup or local delivery within our Rogue Valley service area only.

Any attempt to use this Site to facilitate the unauthorized purchase, ordering, or distribution of prescription medications is strictly prohibited and may be reported to appropriate authorities.

Our website provides a refill request tool as a convenience for existing patients. Use of this tool:

• Does not constitute a prescription transaction
• Does not guarantee that a prescription will be filled
• Does not establish a pharmacist-patient relationship
• Is available for local patients only; we cannot process refills for patients outside our service area
• Requires review and approval by our licensed pharmacy staff before any medication is prepared

All refills are prepared at our White City pharmacy and must be picked up in store or received via our local delivery service. Submitting a refill request does not authorize dispensing; our pharmacists retain full clinical discretion over all prescription fulfillment.

In the event of a medical emergency, call 911 or go to your nearest emergency room.

Some products available at Cascade Pharmacy, including vitamins, supplements, homeopathic products, and natural wellness items, are not prescription medications and are not regulated as drugs by the U.S. Food and Drug Administration.

We make reasonable efforts to keep the information on this Site accurate, complete, and up to date. However, we do not warrant that all content is error-free or current. Product availability, pricing, hours of operation, and services may change without notice.

Our total liability to you for any claim arising from your use of this Site shall not exceed one hundred dollars ($100.00).

Nothing in these Terms limits our liability for gross negligence, willful misconduct, or any liability that cannot be limited under applicable law.

These Terms, together with our Privacy Policy and HIPAA Notice of Privacy Practices, constitute the entire agreement between you and Cascade Pharmacy regarding your use of this Site and supersede any prior agreements or understandings.
